Need some help with documents

My spouse has my family name on her passport. Now what all docuents should I submit for IDENTITY and CIVIL STATUS documents?

1. Birth certificates:
I don't have birth certicate. Will lawyer affadavit work?
My spouse has date of birth on Xth grade marksheet. Is that sufficient?
Do I need to submit original affadavit or copy of that?

2. Legal documents showing name or date of birth changes
For my wife, she has marriage certificate showing oroginal name and changed name. Her passport also has my family name on first page and her father's name on last page. Is this sufficient or any other doc is required?

3. Marriage certificate
I have it so will provide copy of that

In lieu of birth certificate, you can provide secondary or higher secondary school certificate or marksheet that has your date of birth and Parent's name.

A small correction - you can submit the secondary or higher secondary school certificate or marksheet that has your date of birth and either parent's name on it (not just father's name)

I assume you are talking about change in family name. I didn't submit any document in support of last name change of my spouse and I had no issues.
