docs number for open work permit

Hi all!
could you help me in preparing open work permit?
I am doing my application for sponsorship inside Canada, and want to inclue open work permit application together in the envelope.

However it is soooo confusing which document it to be submitted.
There are two check list for work permit. IMM 5556 VS IMM 5488

could anyone please pleae help what will the correct document checklist required for this case?

If you are using the Application to change conditions, extend my stay or remain in Canada as a worker (IMM 5710), then the corresponding document checklist is IMM 5556 (see link below). Please note that it was recently updated.
