Do i need to fill up Representative form??

I have applied thru a consultant [IDP] and got acceptance from Georgian. And the welcome package for the same was send to the Head office of the consultant. But the consultant is not registered with ICCRC.

Do I need to fill up the representative form with the consultant information??
Will it affect my visa approval chances??
Does the embassy contacts the college to verify if I have applied thru a consultant??

just for clarification.
consultants who r not registered member of ICCRC, can only give you advise on course selection.
They can not provide information on immigration and they can also not fill any visa application form including study permit form.

And one more thing... During my diploma, my second year backlog was cleared in third year.
So do I need to submit backlog [KT] marksheets along with other marksheets??
