Im applying for citizenship and paid fees, the receipt has a form asking name, date of birth etc, do I need to fill it out?
 
They need this information for refund cases, so if you apply for family member like dependent children children or spouse if you didn't write the information in the receipt they refund(in case of incomplete applications) will go for each individual named in the application not you who paid the money.
 
Hi!

I need a little clarity, i sent citizenship application for myself and spouse but since i paid with my Credit Card so i wrote my name on the receipt since i was the "Payer". I didnt mention my wife's name on it....

Did i something incorrect ?

As per my understanding it was suppose to be filled by the Payer

that is absolutely right , and if you didn't write any thing and there was something wrong with application cic will refund the money for each applicants 200 under your name and 200 under you spouse name, because you didn't filled the payer section .
