if while submitting your application you werent asked for a medical it means at that point in time medical wasnt needed for your study permit. BUT only if you have after your study permit approval spent 6 months + in a desginated country in last year then you need a medical.
The link you have posted I think means that you need to be BOTH (1) from a designated country and (2) plan to be in Canada for more than 6 months

That's strange because the third condition in that link ends with "or" not "and".

I didn't have to do one for my study permit to get approved, and on my online study permit application, it says that they don't need a medical exam from me.
