Do I have to fill out IMM 5481?

Hi folks,

I am preparing the application to sponsor my spouse and step-son for PR from within Canada (We will live in Canada by the time we send the application). My spouse and step-son are Chilean.

When I get the forms from the CIC page, the form IMM 5481 IS NOT listed among the forms I should fill out, but in the complete guide (Guide 5289) this form IS in the list that I should fill out as sponsor. Also, IMM 5482 is not in the checklist form IMM 5533.

I've thought to fill this form out anyway, just in case, but I would not like to do extra-work that is not necessary.

I'm not sure where you got 5481 from. When I go to Web page and do country specific I do not see that form.
In my opinion 5481 is similar to 5532. And 5532 is on the checklist. Don't be alarmed. Country specific for Chile had no extra forms either.
Anyone else agree or disagree?

In the section that begins with "The sponsor must fill out and sign these forms":

- Sponsorship Evaluation [IMM 5481] (PDF (Portable Document Format), 742.74 KB
(Kilobyte)) (/english/pdf/kits/forms/IMM5481E.pdf), only if you are sponsoring your
dependent child who has no dependent children of their own. Unless instructed otherwise,
sponsors in Quebec do not need to fill this out. See: Sponsors living in Quebec

The 5289 is a detailed guide to all parts of applications possible in sponsorship.

"Detailed information such as how to fill out the forms, is in the Complete Guide (IMM 5289)."
Just because it is in the 5289 does not mean you fill it out.


You said you will all be in canada when sending in your application so your guide to follow will be the IMM 5533.
I input the information and these are the results. (There is no 5481 in your list of required documents. and as it says 5289 is a complete guide)

Chile – Instruction(s) for documents
  • If the principal applicant and the sponsor were married in Chile, submit a copy of the computer printed marriage certificate issued by the civil registry.
  • If you hold a passport from this country, you and your family members included in the application must provide:
    • Certificado de Antecedentes para Fines Especiales,
    • Original “Hoja de vida del Conductor”, even if the principal applicant or the family members included in the application have never been issued a driver's licence.
    For these documents, apply in person at the local civil registry office in Chile.
