My wifes medical will be 1 year old on november 11. I will be sending in her application on sept 26. Am I too late. Does she need to get another medical. Does the expiry date go into affect from the day they receive it or when they get around to looking at it

She will need a new medical. She can wait for the VO to request new medicals or she can get a new medical depending on your VO's timeline in order to reduce the waiting time. Medicals expire after 1 year after they have been done, no matter when they receive your application.

so even though I have one and a half months until it expires I am too late ?

Yes because you need to land before they expire, there's no way you will get through both stages within the time until they expire. Get a brand new one and then apply. That gives your VO enough time to get the application processed.

I was just thinking. With the total waiting time ( sponsorship and visa ) adding up to one year for my wife and I. The new medical could expire before she gets to Canada and they might ask me for another medical. Some countries waiting time is 14 months, How do they get around their visa expireing

Because the VO gets it extended. But your medicals won't make it to the VO before they expire, they will already BE expired. So they cannot extend them.
