Do I fill two separate forms to cancel Representative ?

in the form 5476 part A who would be the Applicant? The Sponsor or sponsored person?

sponsored person is the applicant. He/she is the one who is applying for PR.

Thank you for reply. I'm trying to cancel Representative as sponsor, I don't see that option on the Forms where the sponsor can fill.
