Do document attestation required for Canadian experience class EE

Hi

I have few questions related to Canada express entry PR from Australia. Can someone please help on below questions?

1) Do all documents required to upload need to be attested or not?
2) is PCC from India is good if get it through VFS Melbourne? I read that if you live at multiple location in India you will required PCC from different cities.

No.

Yes.

I read that if you live at multiple location in India you will required PCC from different cities.
That is completely wrong. You need one PCC for the country.

I have one more question : I applied for EE in February, 2017 but application was rejected due to one document missing. This is second time i am going to apply. Can i use same upfront medical completed in February, 2017 as it is not been 6 month or i need to get this done again? I checked with Medical center and according to them any medical is valid for 12 month but they are not sure whether CIC Will accept same or not.
