Your case is somewhat similar to my case.
I was transferred within my group/organization in three different companies. My experience of first two companies matches the 7242 category but not my current organization.
I submitted visiting cards of all my bosses(including current).
I was worried as hell as the situation in my organization is similar and making the current boss think that you are gonna leave is not an option(especially when it takes time). Promotions, apprisal, salary all are affected. I even asked my previous boss for advise and he told that I need not give my current bosses visiting card but I thought lying to CIC is bad, so I asked his business card and told that the process will take around 3 to 4 years and I am going on personal reasons for visiting a relative. I guess you can do the same too.

You should definitely submit your bosses card or info as sometimes they call HR and ask who is the applicant reporting to(I read this here). No need to inform your boss right now. Inform him once your status turns to 'In-process'. You can also write that you cannot give business cards as it would imply you are leaving which will affect your credibiliy in current organization. Its best to say the truth, whatever it is.

There are rules guiding CIC application process. If you cannot meet up by whatever reason please drop the idea and only apply when u can abide by the rules. If u cannot get reference letter from your employer for whatever reason and u cannot offer a letter explaining this, why applying now? Why not 'skip it' and apply when u can?

Well, I have to disagree as even if the reference letter is not available, the applicant should source other evidences like clients letters, testimonials, tax statements, business cards of superiors and peers...etc.

The applicant shouldn't give up easily, especially once some earlier companies don't exist any more due to bankruptcy, merges or acquisitions...etc.
