Different DOB on Birth Certificate

My date of birth (just the month) on my birth certificate is different from my passport and all educational documents.

What is the procedure in this case?

Is affidavit enough and can anyone please share the format for that?

Your birth certificate was issued first. All other documents including your passport came later.

How were you issued a passport with a different date of birth? Was your birth certificate not considered?

No, an affidavit will not work because you cannot have two dates of birth

Thank you so much for the prompt reply.

I only had hospital's certificate with the original month (June). The birth certificate was issued recently based on the hospital data. Unfortunately I live abroad, that is why I am having trouble.

For my passport the National ID card was considered. All the documents have DOB in (July).

I will ask the National registration authority what can I do now.
