Difference between IMM5707E and IMM5645

I have the same confusion in regards to 5707 vs 5645? I am applying online for my mother's Super Visa, who stays in India.

The Document Checklist (IMM 5484) have IMM 5707 as the Family Information form. However, when applying online via GCKEY account, the Family Information form requested is IMM 5645.

Any update from the people who went through this experience? Which is the correct form to upload: 5707 or 5645?
