Did not receive Affirmation of Citizenship form

Hi All,

I received an email this Monday regarding the details of my oath ceremony. The PDF attached mentioned something about the Oath or Affirmation of Citizenship form which should be filled on the day of taking the oath. However, this form was not attached with the email. Will I receive this my mail? Or do we receive this on the day of the oath?

it should be in the same email at the end. If you dont have it, send email back to the address which sent you the email stating the issue.

They should correct it and resend.

Read through the whole PDF. The last page (page 10) is the oath affirmation form.

Hi the oath form is the last page page number 11 and read the pdf attached in your oath invite email.
