As per CIC Document Check list it says that

1. Submit Proof of Full Custody for the children under age 18 and proof that the children may be removed from jurisdiction of the court "

HOWEVER It also says below

2 if the other parent of your children is not accompanying you to canada , you must submit a signed statutory declartion from Non Accompanying parent for minors immigrating to Canada(IMM5604 Oroginal) .



So My Question is if I have a child custody doc (satisfy need of sole custody and Jursidiction), do I still need to have the form signed from((IMM5604) his father(my Former Spouse).


Please reply as I am sure many could benefit from this.

I think that you must have both documents signed.
In my country, if you want to take your children out of country, the other parent must notarize his consent and show it at the border even if we are not divorced, so it makes sense to me.
