Hi
Just wondering if we need to fill out this from even if I (the mother) plan on going back and bringing her into Canada myself once everything is approved. We've been trying to call CIC all day but cannot get through to ask them this ourselves.

The form is required to be signed by the non-immigrating parent.
 
What if that parent is not nor have they ever been involved in the childs life?
My husband (not the childs father) is sponsoring us, I am already on a working visa here.

It doesn't matter. Either the other parent must grant permission for the child to immigrate or you must have court papers granting you total custody.
 
Only if the child is being sponsored to come to canada and is accompanying the principle applicant must you get the other parent to sign under witness by the justice of the peace. And also a photocopy of that parent ID. No matter what you still have to do medical for her or else you wont be able to sponsor in the future. If your daughter is NOT coming with you in the PR app you dont have to fill it out.

For your info i am a sponsor sponsoring husband with his children. We had to get the mother who is not involved to sign it. It was challenging to say the least but we got it. The only thing was the photocopy of their ID which husband wrote a letter that she cannot obtain from her with numerous attempts.

You have to do all deligence to get a signature from him and document your attempts. Also is there custody arrangements? Do you have full custody?

This is for your own protection, lest the non-custodial parent later decide to accuse you of child abduction. Read up on the Hague Convention on International Parental Child Abduction:

An undetermined number of these abductions would be abductions by custodial parents or primary caregivers. Can i ask a follow-up question? If the other parent does sign the form, does it mean that i have the full custody of the child in Canada after I immigrate there? I am asking coz in certain occasions i need to take my kid out of canada to other countries. tks a lot.
