DECLARATION FORM IMM 5669

HI Experts,

I have received a mail from CIC saying I should fill a declaration form IMM 5669 and submit to them within 7 days. I have read previous post where I observed that it is not a strange thing if CIC request for such document.

My question is this, in the section where I am asked to list the addresses of places I have lived in the last 10 years, should I included countries I visited for vacation? Although i spent less than 2 weeks in each of the countries I went for vacation. Or should I just skip those countries?

Please kindly advice and urgent response is needed. Thanks

List only your permanent addresses for each time, don't worry about holidays. A good way to think of it is if you changed your mailing address, include it.

Ensure there are no gaps in your times though.
