Usually IMM5645 is required for temporary visa (student/worker/visitor). However if the officer has assessed that he/she requires you to provide this information it is best to provide it. Its basically family information - parents, siblings, spouse and children, their date of birth, marital status, address, occupation etc.Hi All,
