When listing address & personal history, Q9, Q10, I see the form is in the format of 'From YYYY-MM, to YYYY-MM', so I only put in month and year. Or should we write exactly YYYY-MM-DD?

STick to the format they have given. You wont need to need to be specific when they are not asking for it

AOR is a confirmation mail CIC will send you to say that your documents have been received and they are complete at the first level of review. After that the dcouments will be sent to the nearest processing center to your address.
