I did not write any thing in this receipt. Simply i attached it to an email and send it to cost recovery and visa office as well.
in the email itself i wrote all my data such as uci, application number, full name, dob, ...etc.

-On top of the receipt it says Payer Information and then has the Name of the person whose Card was used.
-?-Client DOB, Address, Phone = Is this for the Primary Applicant or the Payer?
-?-Client ID = Is it the same as UCI ? , if so Primary Applicant and Spouse both have different UCI, whose to mention as this is combined payment of $980(490+490).
-?-At the bottom it says "Payers name on the receipt does not have to be the same as the one on the application" = What does this mean here?
-?-After filling, we scan and send this to the email for Visa Office, right?
