Custodian declaration form uploading

Hi everyone!

My sister is 16, and she need a custodianship declaration form filled and upload, but I cannot find a place to upload it. I kind of scared to waste 150$ just to experiment with writing a LOE, but maybe it is the answer.
I need help asap, because she will derarture next month.

never figured out. I just paid extra to process at the vfs global.
