Current and intended occupation

Hi everyone

Confused with question no 3 and 4 of education/occupation detail in generic application form. Do I have to include the NOC code in current occupation and intended occupation or just to list the position i am holding currently.

PLUS THE GUIDELINE DOCUMENT MENTION SOME WEIRED NOTE ABOUT CHILD ADOPTION OR SOMETIHNG....FOR THIS QUESTION
