Better deal/address with grounds raised on your refusal letter first. So that the same issues will not be the same reason in case you are refused again. The cover letter is actually called LoE (Letter of Explanation). It is not mandatory. It is your way of communicating to immigration officers any supplementary information that is not on the documents uploaded and that you deem important for them to know.

You can just write a simple letter or you can format it like
