Usually it is the letter you receive when you submit your the Biometrics. This is IRCC's way of saying that they received the Biometrics.

If you get this letter first instead of them asking for medical does that means the chances of getting the Visa is slim?????

Medical is not compulsory. It will only be required if needed. Biometrics is important as it contains the applicants identity. If IRCC does not receive it, expect a refusal right away.
