For the question "first became qualified to practice in this occupation" , what date should it be?

Depends on what you do.
Usually it is the date you receive your final degree (date mentioned on the degree).
If you are working on something that requires you to have only a bachelor's, then the date is that on your bachelor's degree.
Check the NOC relevant to your work and read the requirements and duties for that job.
