Consent form issue for Biometric Appointment

I have booked an appointment for a Biometric appointment, but the visa centre shows the list that I should bring the signed consent form.
I am not sure where to obtain one.
Could anyone help out with this issue?

U can download the same from vfs site or u can demand from vfs centre at the time of biometric.

Thank you. I got reply from visa centre as well, they mention the online application doesn't require that form.
