Hello I would like to order notes. I am the sponsor a Canadian citizen living in Canada. My spouse is in Punab India. I am confused about the consent form who fills out section one? Sponsor or principal applicant? Also who fills out section 2 and 2.2? Another thing on my wife's indian passport it has no last name listed, first name only . On the forms I sent I left last name field blank. So, what do I fill in fields where it says last name.? when requesting the notes?

Are you requesting information on your own behalf?
Select Yes it will not ask consent Form - Also Select the Privacy Act it will not ask for any money as well
