confused about two forms 5406 and 5669

Every time i am ready to print the forms thinking I am done, I have more questions.

1. 5406-Does sponsor complete this form? I do not see it on the sponsor checklist.

2. 5669-Does sponsor complete this form? I do not see it on the sponsor checklist.

3. Are there any forms for sponsor or immigrant that are not listed on the checklist but must be completed?

USA married to Canadian born citizen, applying spousal outland and living in USA until receive PR. And going insane over the forms.

1. No...which is why it is NOT on the sponsor's document checklist.

2. See answer to question 1.

3. No. Follow the checklists and you should be fine.


Make sure that you are submitting the latest version of all of the forms, since CIC seems to update at least one or two of them quite often!

Ponga is right - be sure to use the most updated versions and double check for updated ones before you send it or your application will be returned to you.

Just follow the sponsor checklist and the USA-specific immigrant checklist and you'll be fine!
