Not sure if it changed but I was required the IMM5409 form, filled/signed by a Notary Public in my country of residence.

I applied in August last year though, so I am not sure if it is still needed, as canuck_in_uk pointed out.

It still appears in my personalized checklist. Any comments on this? Do I need to still fill ouy IMM5409. My girlfriend is Canadian
