Commercial Invoice

Hello!
I am sending a commercial invoice to CPC-Ottawa for my passport return to the US.
Can someone please tell me a safe unit value to write on the invoice? I was thinking of $300.

Secondly, I received an email from CPC Ottawa to keep my own name in the area provided for Ship From but their 365 Laurier Ave West address. Please has anyone gone through this situation? How did you handle it?
