Hi All,I have been asked to submit Schedule A document for me and my spouse and they have asked for "Client Information: Receipt of information from client" for my kid. I am not sure what information to be submitted for "Client Information: Receipt of information from client". I am clueless what form or document to be submitted for "Client Information: Receipt of information from". Could someone throw details on this?

Did you end up finding what the hell this document is? They asked the same for my parents, it makes no sense.

Client Information: You are required to submit updated residential addresses for yourself and your spouse. Please note: Now that some travel restrictions have lifted we no longer require the proof of travel to quarantine in Canada. This must be received at this office by: 2021/11/17.
