Citizenship:- apply for myself & minor.

Hello Dear,

Currently i am PR & eligible to apply for citizenship. My wife & daughter migrated to Canada in April 2017.
I called CIC to confirm if i can apply for my daughter at the same time and they said " Yes".
However i am stuck on filling out form CIT0003E-2 for minor.

It asked about information for Canadian citizenship certification which i dont have it since i am PR.
SO should i leave it blank or am i filling the wrong form ? Please advise.
