Hello,

1. I want to know, do I need to create a separate online physical calculator accounts for me and spouse.

or

Can I just create a new "Calculation name" for my spouse in my account and create the calculator for the spouse.

2. If I take the print of the physical presence calculator. Only the signature section is getting printed on the second page. Is it OK to sign on the 2nd page, as all the information is on the 1st page?

3. My application has two members. Me and spouse. Should I have to make two document checklists or only one checklist is enough.

In the citizenship application form CIT0002E, # 5 question is: List all the names you have used, including name at birth, maiden name, previously married names,, nicknames.

If I have only one name that I have used all my life, do I still have to fill this question?

I have not changed my name. What should I write for Question 5 of Citizenship application.

My spouse has changed her name legally. What should she write for Question 5.

To be on the safe side, I think you should still answer that question and put your name in the table and list it as 'name at birth'. Even though I think it's for people who had a different name at birth and changed it later on. But according to the sticky thread post in this forum by @rajkamalmohanram which serves as a comprehensive guide, he suggests that everyone should write their name as name at birth. See below.

Q5: List all names that you have used, including name at birth, previously married name(s), alias(es) and nicknames.
  • Whether you have a nickname / alias or not, everyone MUST answer this question with their name at birth.
  • If you have used an alias / nickname or any other kind of name, then answer truthfully.
