For question 3B "Request for a different name"

If I don't wish the name changed. should I leave the answer blank? or do I copy the same exact name from 3A

still can't figure out if the expectation is to write my name or write NA?

Should I do this:
3A) Smith John
3B) NA
3C) NA


3A) Smith John
3B) Smith John
3C) NA

Frankly I wanted to leave the 3C blank, as I have no other names

Also another question

In the address.works experience section Can I write the last address/work as Actual Start Date until Now/Present Time, or do I need to specify the exact date I sign the application?

Can it be?

or it has to be:

From To Work/Education Employer or School City, Country
2013-01 2016-08 Work J.S and Company Toronto, Canada

I would not recommend you to leave it blank. strictly follow the guidelines provided by Citizenship form guide and you will have peace of mind.

If you do bit of research on the forums, i found that moment you enter anything in 3B and 3C they have got letter with enquiry for proof of documentation regarding the name written there as 3B and 3C is for Name Change cases generally applicable to women changing their last name after marriage.
The cases were the application got enquiry letter later submitted to withdrawn change of name and then there application was processed just causing delay in application.

If you still have doubt, best to Call CIC call centre and follow their directions. The agents are more than happy to assist.

Is it only applicable to temporal workers who came to canada before PR?
