Guys, I have a request: I am not able to find contents of CIT 0049 oath of citizenship form anywhere on internet. Is it possible for someone to share the content of the form or a blank form that I can refer ?

Did you not get the CIT 0049 in the pdf along with the email for ceremony invite? (Last page of the pdf)

1 field is about oath and the other is confirming that you have no criminal proceedings
