Sorry to hear your situation. I can give you an idea, but I am not sure if it is legal to do so. I am not a lawyer and I do not take responsibility of what I say here. I also do not understand why you need Canadian visa.
Anyway, here is my idea for you: find a friend/family that you can trust and has a permanent address in the US and is willing to help you. Provide that address to CIC Buffalo and go home. Buffalo will send you a letter requesting passport for PR visa stamp in a couple of months to that address (friend/family address). They open it, read it and let you know about the letter. You send your passport to your friend/family first and they will mail from their address to Buffalo for visa stamp. Your friend/family get your passport back with PR visa in it. They will mail the passport to you to your home country. You buy a ticket to Canada and land there directly from your home country before your medical expires (I think 12 months from the date you did your medical).
You are in a good position that you have already done for your medical in the US. The next step is passport request for visa unless you have some medical issues (which your doctor should have told you by now). If Buffalo wants to interview you, then you would have to come back to the US, but that is less likely to happen for PNP people (I am assuming you are a pnp). I hope this helps.
