Hello,

I received thses two files from Rome CIC. Why do they need this info? i have already filled out a similar form with the original sponsorship package. Anyway, the real question is, do i fill them for my husband or me, or both, and can i send them via email? or regular mail?

Sounds like they want your Personal History again from the IMM5669 Background form. Can you share exactly what the request stated?
Are you sure you didn't leave any gaps in your residence or personal history charts?

Since it's coming from Rome office, they would most likely only care about the applicant and not the sponsor.
 
they're most likely asking for it because the information you submitted first was not precise enough, or maybe had gaps. It's important for the applicant to have a precise history with all periods accounted for. It's to be able to do the background checks.
So - only for the applicant, and I would suggest that you send them by post, but also send them a scanned copy so they can start reviewing it.

My wife was phoned by a visa officer and was asked to resubmit a form about chronological personal history.
There were apparently gaps.
These gaps were not explained.
My wife is 32. The 5669 form asks for info from age 18 or the last ten years. Whatever comes first. In her case, it's the last ten years then, right? Is this the missing pieces?(from 18-22?)

Another thing: my wife had multiple activities with work and studies similtaneously in her personal history. WHAT is the expected format? Do we separate dates by activity?(single out each thing) or do we try and put a block of time and list different multiple activities? I thought we had given all info for what they asked about.
What pisses me off is that you can't contact the officer by phone. You have to wait for them to reply to an email. A simple phone call would help me to clarify exactly what they need.
 
Question 8 of IMM 5669 (edition dated 12 - 2012; which I think is the most recent edition of this form) does not say <since the age of 18, or the past 10 years, whichever comes first>. The last 3 words quoted were changed in this latest edition to <whichever is most recent>. So I do not think that a 32 year old wife has to give an account of her personal history from the age of 18 to the age of 22. An account covering the period when she was between the ages of 22 and 32 answers the question. There must be other gaps in her personal history?
