Choice of Timing of medical exam

Understanding that medical exam is conpulsory, but bases on the webpage, it said there are two way of doing so, one is to wait till you receive the instrucution after submission of appliacation, the other one is to do the upfront medical check prior to submission, depending on your category, without not specifying which one. I am BC PNP EE under international graduation( with job offer). Can someone kindly inform me if I may do it after submission of application?
