No, that form is to add dependents to your application.

You should mention the child on the Additional Family Information form, and include a letter of explanation, saying that you have not included your child in the application because he/she is a Canadian Citizen, include a copy of the birth certificate.

I was advised (by a specialist, not a lawyer) not to include my Canadian born daughter on any forms other than on IM5406 Additional Family Information form. CIC sent me an e-mail asking for a copy of her birth certificate which I sent and that was that. The child doesn't need to appear anywhere else since they do not require PR status to remain in Canada. You might want to send a copy of the birth certificate with the application just so they have it and don't need to ask later.

I have one more question around the same topic. I have a son who is citizen of Canada. In the main form(IMM 0008) Question # 3 do the total count should include him too??
