Child Passport Application PPTC445

Hi,
I had applied for my child's passport application. However, they returned it and asked to fill out PPTC 445 with the application. I could not find PPTC 445 anywhere. Can you please kindly inform me where I would be able to find PPTC 445?

Hi, can you guide me through the process? My spousal application is in process and most likely baby will be delivered out of Canada. My husband is a Canadian national.
