Child crossing border with non-custodial parent

Hi - my husband's 11 year old daughter wants to come visit us this summer in Canada from Portugal, and we were planning to take her to Disneyland as well. What kind of documentation do we need from her mother (whom she lives with full time) to allow her to cross the borders with us without any problems?

notaried document setting out legal parents name, child's legal name and date of birth.....travel plans....i.e. portugal to canada dates arriving and leaving and airline....legal name of people with whom she will be with and their address.....dates that you will be entering USA and leaving USA and by what method...i.e.plane/train.......where you are going in the USA..specific place and dates......
