We are having PR status but came to my home country Pakistan due to some personal issues. My daughter has born and now we are planning to moving back to CANADA. What is the procedure and required forms and where to apply for the travel document of new born. Urgent response would be appreciated.

Your baby does not qualify for a travel document, and will require a TRV or TRP to enter Canada. Once you are in Canada, you can sponsor your child for PR.

So its mean we can not travel as combine family. Is there no option to carry her?

As explained above, you need to try applying for a TRV.
