Pfl is procedural fairness letter......is usually a letter to give the applicant a chance to provide more evidence before a decision is made.
A quick google search tells me that A40(1) is the provision under immigration act that deals with "inadmissible for misrepresentation". Your notes might have more details about the concerns raised by the visa officers. More details in the following document
