i have doe rukhsati already and send them paper in dec,2011 already ....
which letter they send me they not saying that they belive that relationship is not genuine.

Send your Rukhsati pictures and proof again, tell them that marriage consumption has been done as your wife stayed with you when she visited you. Tell them that you will appeal the decision if Visa is refused. Sometimes they just want to see your response and see if you are confident answering this question.

I just received a request to submit Supplementary information form from CHC Islamabad. Do I have to fill it out with hand or make a word document and type-in the information.

Check the tracking of the courier and see who received items (Nasir or Danish). If one of these guys received your documents then it means your documents have reached safely.
