Which option did you pick CAIPS notes or Overseas visa file with CAIPS notes ?

bro canpak, wish u gud luck BUT when u come pk ..jus avoid rikhaw polution . see ur doc before visiting and drink NESTLE water in pk yaar .u will be having no prob. its wat i did before leaving family in canada...

Thanks for your advice saifsmarty. I will be seeing a doctor before going to Pakistan, its the summer heat that is a disadvantage for me in Pakistan as I am not accustom to hot climates, hence my trips always are planned for the fall.

Form IMM5563 : Access to Personal Information Form

Only these sections needs to be filled
1: Surname
2: First Name
3: Date of Birth
4: Access to Information ($5.00) - Only select Receive copies of the records.
5: Privacy Act(no Fees) Section - Only in English needs to selected
6: Request FOR- Demande Pour SECTION- Select only Caips
7: REQUIRED INFORMATION-RENSEIGNMENTS NECESSARIES SECTION - Only Embassy Name and Immigration file number to be mentioned
8: Request Detail by Canadian Citizen Section - Name, Phone Number, Address, Date & Signature of the person who is requesting CAIPS on behalf of you.


FORM IMM5475: Authority to release Personal Information to a Designated Individual

Only these sections needs to be filled out

1: Only Select " I Authorize Citizenship and Immigration Canada and Border......to the following individual"
2: Section (1-4) Give all your details: Full Name, Date of Birth, Embassy Details & Immigration file number
3: Section (5-6) All Details of your designated individual who is in Canada. Designated Individual can be your spouse or anyone who u know in Canada. Give all his details Name, phone number, mailing address, email address etc
4: Section 7 : Applicant Signature and Signature of your Spouse or Common-Law Partner


If you are in Moscow then the applicant should fill the form and send the scanned copies to the person in Canada so that after signing it he/she can mail it.


In the end the person in Canada should attach one draft of $5.0 with these document the cheque/draft is payable to the "Receiver General of Canada" . Also needs to be attached in the copy of his/her Canadian Citizen Ship Card or Canadian Passport Page of your spouse or Designated Individual with this application.

ya,u rite bro canpak.we become use to of it where we live for few years.
its my personal feelings and experience :

i was more stronger and used to face every hard weather when i was living in pk before going to canada.
in canada ppl r more sencitive.
well,depends upto atmosphere,living standards,culture etc.

I was told that they will photocopy the file from Islamabad and send you that.... maybe I can call the number for Ottawa and ask...

Donot drink soft drinks that are in glass bottles only use plastic bottles... Definately use bottled water (prefere Nestle as suggested) since day 1, make sure you take malaria pills with you see your doctor for that and he might ask you to take a shot. I was born and lived there for 15 yrs but when I went there for the first time after 5yrs I got really sick and was on bed for 2 weeks and that was in June. Avoid cheap outside food aswell only eat in restaurants if you really want to have something from outside Good Luck!!!

I had read in previous forums that they send you the file, was not specific if it was original or copy. I have adjusted my post and wish that someone with more knowledge of process can clarify.

and most importantly take 2 imodium pills everyday and your stomach will stay strong, thats what I do and never get sick and yes I get Malaria and Hepatitis injection before I go too...

I have done some more searches on the topic. What i have found is that one wants to select the option to receive copies of the records. With that if you select CAIPS only, all updates on the status of the file entered in the computer system will be printed. If you select Overseas visa file, they will send you a copy of the file along with CAIPS that will take longer then just receiving CAIPS due to time for pulling file to copy.

For us, I copied the complete file before sending to CHC, so I have all that they would send.
