CHC ISLAMABAD HAS SCREWED UP WITH MY MEDICAL CALL!

Salam guys. I recently got my medical call and I was very happy that i got it so quickly. My happiness did however disappear soon after my friend told me that it had a problem with it. My medical call package includes two pages i.e the checklist and the address of the medical centers. It doesn't include the medical letter it is supposed to have, which I am supposed to submit to the medical center.

What to do now?

mail kr de aur agr medical ki date tk reply na ae tb bhi medical k liye zaror jana mail ka print le kr....

Bro They are Sending Only 2 pages One Is checklist WHich Includes Your file Number And on the Very NExt Line There's a tick On medical check

And Other Medical adress Thats it Even i got it Same these 2 pages Letter And Am done with my medical Today They accepted that Document

previously they send medical form with medical call which is called as form A but i think they a not sending any medical form

Yar i would have never noticed this but, a friend of mine took the medical screening last Monday and he got the three pages (that I personally saw) and yesterday he pointed that out to me. So that's what got me worried. khair will have to make a calls here and there to confirm. Your response has sorta cheered me up thanks

which checklist??if it states ur file number ,name ,and date of birth on front page,then it is absolutely fine,just book ur appointment,with medical office.
