Original BAY Form from Nadra, translated for Child
DOB copy of parents, notarized
Authority letter that the child can travel without father.

Photocopies of all of the above supporting docs, notarized.

anyway, can u tel me wen the chc isl wil contct for file number??
how much time they take for passpirt nowadays???

They sent u a tcs letter with acknowledgment letter
N pp takes almost 6-8weeks.


Can somebody help me please Islamabd returned my baby's application
N said to change surname
I'm suppose to fill up new forms or sent old ones with changes ?

Are all these forms required if the child is a Canadian Citizen born in Pakistan to Afghan Mother who is being sponsored to Canada and they have the passport of the mother ready for issuance?

What is a DOB that you mention?
Also Original Citizenship Card, how is that possible if the child has not gotten Canadian Citizenship yet.
