Hello, I have some questions to ask to you guys, but first let me give you the background story. I obtained my PR card roughly 8 months ago. However, the name on the PR card is showing only my First and Middle name, but not my last name. Let's say my name is Jack William Johnson, instead of showing "Jack William Johnson", my PR card is only showing Jack William (my middle name becomes my surname). I know that CIC made my profile based on my legal documents and passport from back home, I found out that my original passport from back home says "Jack William" and on the back page it says "Also known as: Jack William Johnson". Is there anything I can do to change my name on my PR? Could you please provide me with a link or the forms I need in order for me to appeal for a name change on my PR card? Thank you.
