Changing conditions on Open Work Permit

Hello.
I'm already in Canada and I want to apply for changing conditions on my open work permit (I didn't provide medical exam and couldn't work in health field, now I can provide it). I don't work right now.
I need some clarification on form 5710. On the 3 question what should I choose?
Check the appropriate box or boxes to indicate if you are applying for:
1 - A work permit with the same employer
2 - An initial work permit or a work permit with a new employer
3 - Restoration of your status as a worker
4 - An initial or an extension of a temporary resident permit

either 1 or 2 depending on whether you're planning to change employer. Not much difference when applying for an Open work permit

Well, thanks. Number 2 looks like the most relevant, but still - I don't work so I don't have any employer. Any employer will be new for me =)
