chances of rejection after final decision?

hello i received my gcms notes yesterday and here's my assessment section.

Final: Approved
Counterfoil Req: Yes

but i am yet to receive a final decision on my gc account. does the above notes indicate that a final decision has already been taken on my application and if yes can it change?

Is there OPR in outgoings? Also, how does your first page of notes looks like if u dont mind sharing.

I don't think i will get a passport request since i already submitted my passport with my application so there isn't any OPR on my outgoing section. also, i don't see anything useful on my first page other than the visa office handling my application and some client id number. i can't seem to upload a picture here, sorry.
