I just want to share that me and my husband already receive our visas today, but still, wala pa yung sa daughter namin.
Hopefully within this week we will also receive her TRV soon.
For those who would also apply in the future and are always browsing through this forum, I would like to share the set of documents
I have submitted
(NOTE: I am NOT saying that these documents would guarantee approval of visa, I am just sharing what I have submitted):
1) Cover Letter addressed to the Canadian Embassy (Visa Section)
2) Application for a Work Permit - Checklist
3) Job Offer with my latest résumé.
4) Labor Market Opinion (LMO) confirmation provided by Human Resources & Skills Development Canada (HRSDC)
5) Completed IMM 1295: Application for A Work Permit
6) Completed IMM 5465: Family Information Form (for all applicants 18 years of age or over)
7) Completed PIF 2011-03: Work Permit Additional Information Form
8. Original Passport (old and new passports showing history of travel)
9) Copies of the passport bio-data page
10) Evidence that the requirements of the job offer are met:
(10a) Certificate of Employment and Compensation (original)
(10b) Detailed Description of Current Job (certified true copy of Job Description by the DFA)
(10c) Secondary Education Academic Credentials (all original documents)
* Diploma
* Certification of Graduation
* DepEd Form 137: Secondary Pupil's Permanent Record
(10d) Post-Secondary Education Academic Credentials (all original documents)
* Diploma
* Certification of Graduation
* Form No. IX: Official Transcript of Records
11) Original NBI Certificate authenticated by the DFA
12) Two passport photographs (for me and each family member)
13) Manager's check for the processing fee (PhP 6450, one payment for each person)
14) Proof of Relationship
14a) Birth Certificates (including spouse's and eligible dependent's)
14b) Marriage Certificate
15) Proof of funds / source of funds (all original documents)
15a) Bank Certifications (Passbook of Time Deposits-PhP and USD, Special Deposit Accounts-Investments)
15b) BIR Form No. 2316: Certificate of Compensation Payment/Tax Withheld
15c) Employee Status Report (showing the income movement from 2010 - 2012)
15d) Photocopy of car registration
And I think it is also important to note that for my husband who applied for Spousal Open Work Permit (SOWP),
he did
NOT submit the following (I am also sharing this so that others who are preparing for SOWP, they will not
waste time for these documents which are not necessary):
1) Evidence that the requirements of the job offer are met:
(1a) Certificate of Employment and Compensation (original)
(1b) Detailed Description of Current Job (certified true copy of Job Description by the DFA)
(1c) Secondary Education Academic Credentials (all original documents)

Evidence that you meet the requirements of the job offer, e.g. employment references outlining previous jobs and job duties, copies of relevant education certificates, proof of professional qualifications, originals of all school transcripts (starting at the secondary level) certificates, diplomas, degrees, etc.
**Note this is not required if you are completing an application for an open work permit as an accompanying family member.
Tomorrow, we will submit our resignation letter and we will proceed to POEA on Thursday.
